What hemoglobin measures
Hemoglobin is the amount of oxygen-carrying pigment in your blood, reported in grams per decilitre (g/dL). Each red blood cell is packed with hemoglobin molecules, and each one grabs oxygen in the lungs and releases it in the tissues. So the hemoglobin value is, quite directly, a measure of how much oxygen your blood can carry — which is why it's the headline number when anemia is in question.
Because hemoglobin lives inside red cells, it tracks closely with the other red-cell numbers: the hematocrit (the percentage of blood that's red cells) and the red-cell count (RBC). But hemoglobin has a subtlety in common with them — it's a concentration, so it depends not just on how much pigment you have but on how much plasma it's diluted in. Dehydration concentrates it; over-hydration dilutes it.
Like the other red-cell numbers, hemoglobin is nonspecific on its own — it tells you oxygen-carrying capacity is off, not why. Its power comes from being read alongside the MCV (red-cell size), the RDW (how much cells vary), and markers like ferritin.

The key caveat: a hemoglobin right around 13.5 is one of the more over-read results on a CBC — it's the very bottom of the normal male range and normal for women, not automatically a problem. A hemoglobin of 13.5 in a well woman reads very differently from 13.5 in a man whose level is falling test-on-test. And because hydration shifts the value, a mildly high or low result is read in that context. The pattern, not the isolated number, carries the meaning.
What a low hemoglobin means (anemia)
A low hemoglobin is the definition of anemia — less oxygen-carrying pigment than normal. The MCV helps sort the cause by red-cell size:

- Dehydration — the most common everyday cause; concentrating the plasma raises the value without any real change in red-cell production. Rehydrating often brings it back down.
- Smoking and living at high altitude — both push hemoglobin up as a normal adaptation.
- Chronic lung or heart conditions — low oxygen drives more red-cell production.
- Polycythemia vera — a less common bone-marrow disorder; usually a markedly high, persistent value.
