Understanding lab results

Hematocrit — often shown as HCT or PCV on a blood count — is the percentage of your blood that's made up of red cells. It's one of the numbers people notice most, because it's easy to flag high or low. Here's what it measures, the typical range by sex, what specific values mean, and when a high or low hematocrit is worth a closer look.

What hematocrit measures

Hematocrit is the fraction of your blood volume made up of red blood cells, reported as a percentage (or sometimes a decimal — 0.45 means the same as 45%). Spin a tube of blood in a centrifuge and the heavier red cells settle at the bottom; the hematocrit is simply how tall that red-cell column is compared with the whole sample. The rest is mostly plasma, the straw-coloured liquid that carries the cells.

Because it's a proportion, hematocrit depends on two things: how many red cells you have, and how much plasma they're floating in. That second part matters more than people expect — if your plasma volume drops because you're dehydrated, the same number of red cells now takes up a bigger share, and the hematocrit reads higher even though nothing about the cells has changed.

Like the other red-cell numbers, hematocrit is nonspecific on its own. It tells you the balance is off, not why. Its value comes from being read alongside your hemoglobin, your red-cell count (RBC), and — when anemia is in the picture — indices like MCV and RDW.

The key caveat: a hematocrit right around 50 is one of the more over-read results on a CBC — it's the very top of the normal male range, not automatically "high." A hematocrit of 50 in a well-hydrated, healthy man with a normal hemoglobin reads very differently from a hematocrit of 56 that's climbing test-on-test with a high hemoglobin. And because dehydration alone can lift the number, hydration on the day of the draw is part of the interpretation. The pattern, not the isolated number, carries the meaning.

A borderline hematocrit on its own, with an otherwise normal count, is common and usually not urgent — especially if hydration on the day could explain it. It's worth a clinician's interpretation when the hematocrit is clearly high or low, when it's paired with an abnormal hemoglobin or RBC, when it comes with symptoms like fatigue, breathlessness, dizziness, or pallor, or when it keeps shifting across repeat tests. A markedly high value that persists after rehydrating, or a low value with symptoms of anemia, both deserve a proper look.
