What free T4 measures
Free T4 is the fraction of thyroxine that's unbound and available to your tissues. Your thyroid makes thyroxine (T4), most of which travels through the blood stuck to carrier proteins; only the small free fraction is active. The free T4 test measures that active portion, which is why it's preferred over "total T4" — it isn't thrown off by changes in the carrier proteins (as happens in pregnancy or with certain medications).
T4 is best thought of as the storage hormone: the body converts it into the more active T3 where and when it's needed. So free T4 tells you how much thyroid hormone is available, while TSH tells you whether the body thinks that amount is enough.
Free T4 is rarely interpreted alone. Its meaning comes almost entirely from being read alongside the TSH — and, when an overactive thyroid is in question, the free T3.

A common adult reference range is:
- ~0.8–1.8 ng/dL (about 10–23 pmol/L)

What specific free T4 numbers mean
People often arrive with one figure in mind — a free T4 of 1.2, or 0.8, or 2.0. The bands below are a rough guide to how far outside the typical range a value sits, not diagnostic thresholds. What the number means depends heavily on your TSH, your symptoms, and the trend over time.

The key caveat: a free T4 anywhere in the normal band means little without the TSH next to it. A free T4 of 1.2 with a normal TSH is a normal thyroid; the same 1.2 with a TSH of 20 tells a very different story. The TSH–free T4 pattern, not the isolated number, carries the meaning.
What a low free T4 means
A low free T4 means less active thyroid hormone is available. The interpretation follows the TSH:

Symptoms of an underactive thyroid — fatigue, cold intolerance, weight gain, dry skin — often accompany a genuinely low free T4, and it's read in that context.
What a high free T4 means
A high free T4 means more active thyroid hormone is circulating. Again the TSH sets the frame:
- Low TSH + high free T4 = hyperthyroidism — an overactive thyroid (for example Graves' disease or a hot nodule).
- Medications and supplements — too high a dose of levothyroxine, or some other drugs, can raise it.
Symptoms of an overactive thyroid — palpitations, weight loss, heat intolerance, tremor — often accompany a genuinely high free T4, and free T3 is usually checked alongside.
