What the A/G ratio measures
Your blood carries two broad groups of protein: albumin — the single most abundant, made by the liver — and globulins, a mix of antibodies and various carrier, clotting, and transport proteins. The A/G ratio simply divides one by the other. Because the two groups shift for different reasons, their balance is a useful pointer: is a change in total protein coming from the albumin side (often the liver or kidneys) or the globulin side (often the immune system or inflammation)?
Labs rarely measure globulin directly — they calculate it as total protein minus albumin — so the A/G ratio comes automatically with a standard panel. Like other derived values, it's nonspecific: a starting clue, read with the actual albumin and total protein.

What a low A/G ratio means
A low ratio means globulins are high relative to albumin. That can happen three ways — globulins up, albumin down, or both:
- Chronic inflammation or infection — raises globulins (antibodies and inflammatory proteins).
- Liver disease — the liver makes less albumin.
- Kidney conditions — albumin can be lost in the urine.
- Less commonly, conditions that raise antibody proteins, which is why an unexplained, clearly low ratio is sometimes followed up with protein electrophoresis.
A mildly low ratio is common and nonspecific; a clearly low or persistent one is what prompts a closer look.
What a high A/G ratio means
A high ratio (albumin high relative to globulin) is less common and usually less concerning:
- Low globulins — some immune or genetic conditions.
- Dehydration — concentrating the blood can nudge it up.

A low ratio with a high total protein points toward the globulin side (inflammation/immune); a low ratio with a low albumin points toward the liver or kidneys. It's worth a clinician's interpretation when it's clearly low or high, persists across repeat tests, or appears with an unexplained high globulin, low albumin, or symptoms.
