Specific gravity is the urinalysis square that measures concentration — how much your kidneys packed into the water they released. It's reported as a density ratio against pure water (1.000), which is why the numbers look so oddly precise: 1.010, 1.020, 1.025, 1.030. Your kidneys constantly decide how much water to keep. Concentrated urine (higher number) means they were conserving water; dilute urine (lower number) means they were shedding it. That makes it two things at once: a rough hydration gauge, and a context line for every other square on the strip. Two honest observations about this table: the differences between neighbouring values are mostly when you last drank, not health versus illness; and single values matter far less than patterns across repeats.
The two patterns that actually matter
- Fixed at ~1.010, every time. Urine locked at the same concentration as blood plasma (called isosthenuria) regardless of hydration can mean the kidneys are losing their concentrating ability — one of the quieter early signs of chronic kidney disease. It's the repetition, not the value, that signals: 1.010 once is a hydrated Tuesday; 1.010 on every sample for months is a kidney-function conversation.
- Persistently very dilute + very thirsty + large volumes. The combination (not any single piece) is the classic diabetes insipidus picture — rare, and diagnosed with proper testing, not a dipstick.
What fakes a high number
Specific gravity assumes the dissolved stuff is the usual salts and waste. Three things break that assumption:
- Glucose — diabetic glycosuria adds real density; a high number with a positive glucose square is about sugar, not hydration.
- Protein — significant proteinuria does the same.
- IV contrast dye — recent imaging with contrast produces impressively high readings for a day or so, famously confusing.
There's also a measurement wrinkle: dipstick pads estimate specific gravity chemically, while lab refractometers measure it physically, and they can disagree slightly — one more reason a lone surprising value gets repeated rather than acted on.
When to talk to a doctor
- Persistently ≥1.030 despite drinking normally, or with positive glucose/protein squares — worth reviewing rather than re-hydrating past.
- Fixed ~1.010 across repeat tests — ask about kidney function (creatinine/eGFR).
- Very dilute urine with genuine excessive thirst and urine volume — a real workup question.
- A single high or low value in a well person — almost always hydration; repeat at another time of day before worrying.
