pH is the urinalysis square that measures how acidic or alkaline your urine is — and unlike blood pH, it's designed to wander. Your kidneys push acid or base out precisely so your blood doesn't have to move. Here's what 5.0, 6.5, 7.0 and 8.0 actually mean, the famous old-sample artefact behind many alarming 8.0s, the kidney-stone connection, and the few patterns that genuinely matter.

What the number measures
The kidneys balance your blood's acidity by excreting the day's acid or base load into urine. Urine pH is a snapshot of that housekeeping: physiologically anywhere from about 4.5 to 8.0, typically 5.5–6.5, moving meal to meal. That's the first honest thing to know: a single urine pH is one frame of a moving picture — labs read it as context for the other squares, crystal risk and infection clues, not as a verdict.

The famous artefact: the old sample
Urine left standing at room temperature is a culture flask. Bacteria — contaminants or real — split urea into ammonia, and the pH drifts steadily alkaline. A high pH on a sample that travelled or waited means very little, which is why an unexpected 8.0 in a well person earns a fresh, promptly tested repeat before any interpretation. When alkaline urine means something
- Infection with urease-producing bacteria — organisms like Proteus make urease, split urea and alkalinise urine. The tell is company: positive leukocyte esterase, nitrites, an elevated white-cell count, symptoms. These infections also build struvite stones — the "infection stones" that grow in alkaline urine.
- Renal tubular acidosis — an uncommon condition where the kidney can't acidify urine properly: persistently alkaline urine while the blood runs acidic. A clinician-level diagnosis made with blood tests, never from a dipstick.
- Medications — sodium bicarbonate, acetazolamide and citrate supplements alkalinise by design.
The kidney-stone connection
pH decides which crystals can form. Acidic urine favours uric acid stones (management often deliberately alkalinises urine with citrate). Alkaline urine favours calcium phosphate and infection-driven struvite stones. This is the main setting where urine pH is actively managed rather than just observed — stone formers often track it on purpose, and trends across weeks beat any single reading.
When to talk to a doctor
- Alkaline pH + infection signs — white cells, nitrites, symptoms — is a UTI conversation, and the organism matters.
- Recurrent kidney stones — urine pH becomes a management tool; ask rather than self-adjust.
- Persistently extreme values on fresh samples without dietary explanation — worth a look (renal tubular acidosis is rare but real).
- A lone 8.0 in a well person — fresh-sample repeat first; most disappear.

What does urine pH 6.0 or 6.5 mean?
Textbook territory — mildly acidic, where most healthy samples sit. Nothing about 6.0–6.5 needs explaining; it mostly serves as context for the rest of the urinalysis and, in stone-formers, for judging crystal risk.
What does a low urine pH like 5.0 mean?
Firmly acidic. Common with high-protein diets, after fasting or in ketosis, with diarrhoea-related acid load, or simply overnight (first-morning urine runs acidic). It matters most for people who form uric acid kidney stones, which crystallise in acidic urine — one reason clinicians sometimes deliberately alkalinise urine to treat them.
Can diet change urine pH?
Yes, within hours — it's one of the most diet-responsive numbers on the dipstick. Meat and protein push it acidic; fruit and vegetables push it alkaline (citrus fruits too: their citrate is metabolised to bicarbonate). This responsiveness is why single values are read loosely, and why 'alkaline water' marketing leans on a number your kidneys were always going to move anyway.
Can a urine pH result be wrong?
The classic error is the standing sample: urine left at room temperature grows bacteria that split urea into ammonia, drifting the pH alkaline — an 8.0–9.0 on an old sample means little. Dipstick pads also read in half-unit steps, so small differences between tests are noise. A surprising value is normally just repeated on a fresh, promptly tested sample.
