Neutrophil-to-lymphocyte ratio (NLR): what a high NLR means
The neutrophil-to-lymphocyte ratio (NLR) is a simple number you can work out from a standard blood count — a rough gauge of physiological stress and inflammation. Here's how it's calculated, the typical range, what a high NLR means, and why it's only meaningful in context.

What the NLR measures
The NLR divides your absolute neutrophil count by your absolute lymphocyte count — two numbers already on a standard CBC with differential. Because these two white-cell types tend to move in opposite directions under stress (neutrophils up, lymphocytes down), their ratio is a compact way to capture how much physiological stress or inflammation the body is under. It's used as a quick, inexpensive signal — and, in research, as a prognostic marker across many conditions.
Like other derived values, it's nonspecific: a raised NLR says "there's stress or inflammation," not where or why.
How to calculate it
NLR = absolute neutrophils ÷ absolute lymphocytes (both in x10^9/L).
For example, neutrophils of 5.0 and lymphocytes of 2.0 give an NLR of 2.5. Use the absolute counts, not the percentages.
Typical range
In healthy adults:
- ~1–3 — typical
There's no single universal cutoff, and it varies by lab and population, so treat these as general guides and read your value in context.

The key caveat: the NLR moves quickly with whatever the body is doing, so a single high value can simply catch you mid-infection. The trend and the underlying counts carry the meaning — a spike that settles is very different from a persistently high ratio.
What a high NLR means
A high NLR usually reflects the ordinary stress response:
- Bacterial infection — classically raises neutrophils and can lower lymphocytes, pushing the ratio up.
- Acute stress, illness, injury, or surgery — physiological stress shifts both cell lines.
- Corticosteroid medication — raises neutrophils.

What does a low NLR mean?
A low NLR (neutrophils low relative to lymphocytes) is less commonly the focus. It can appear with some viral infections (which raise lymphocytes) or when neutrophils are reduced. Like a high NLR, it's read in context with the full differential.
