Alkaline phosphatase is an enzyme that turns up in several tissues — most importantly the liver and bile ducts, and bone. Smaller amounts come from the intestine, kidney and, in pregnancy, the placenta. The blood test measures the total of all these sources combined, which is the single most important thing to understand about ALP: a raised value doesn't, on its own, tell you which tissue it came from.
That's why ALP is rarely interpreted alone. Its meaning comes from being read alongside the other liver enzymes — especially GGT, which helps separate a liver source from a bone source — and from the person's age and situation, since growing bones and pregnancy naturally raise it.

The key caveat: an ALP of 120 or so is one of the more over-read results, because the "normal" ceiling varies so much between labs (120 in one, 147 in another) and because children, teenagers and pregnant people are expected to run high. A value near the top of the range in a healthy adult with a normal GGT reads very differently from the same number that's climbing with a high GGT. The source and the context, not the isolated number, carry the meaning.
What a high ALP means — and where it comes from
A high ALP means more enzyme is circulating, and the first job is to work out the source:
- Liver / bile ducts — often blocked or slowed bile flow (cholestasis), from gallstones, a blockage, or certain medications. This usually raises GGT as well.
- Bone — active bone turnover: growth in children, a healing fracture, Paget's disease, vitamin D deficiency, or an overactive parathyroid. GGT is usually normal here.
- Physiological — normal in growing children/teens and in pregnancy (placental ALP).
