MPV — mean platelet volume — is the average size of your platelets, reported with every CBC in femtolitres. It may be the most over-worried-about number on the panel: it rides along automatically, varies between labs, swells in the sample tube, and means almost nothing by itself. Here's what high, low and normal MPV actually indicate, the one combination clinicians genuinely read, and the artefacts behind many scary-looking results.

What MPV measures
Platelets are the cell fragments that plug leaks in blood vessels. Your bone marrow releases them large, and they shrink as they age over their roughly 7–10 days in circulation. MPV is the average size across your platelet population — which makes it a rough gauge of how young your platelet supply is:
- Bigger average (high MPV) → more fresh platelets → the marrow is releasing briskly.
- Smaller average (low MPV) → an older population → production is slower.
That's genuinely all it measures. Everything else is inference — and the inference only works together with the platelet count.

The lab-range caveat matters more here than almost anywhere on the CBC: MPV reference ranges differ noticeably between analysers (commonly somewhere in the 7.5–12 fL neighbourhood), so "high" on one report can be unremarkable on another. Your own report's range is the one that counts.
The cardiovascular-risk claim, honestly
You'll find pages connecting high MPV to heart attacks and clots. The kernel of truth: population studies have associated higher MPV with cardiovascular events, because larger platelets are more reactive. The honest rest: the effect is small, confounded, analyser-dependent — and no guideline uses MPV to assess an individual's cardiovascular risk. If heart risk is the question, the numbers that actually inform it are cholesterol and its ratios, blood pressure, HbA1c and smoking status — not platelet size.
The sample artefact
MPV has a famous preanalytical quirk: platelets swell in the EDTA tube while a sample waits for analysis. A blood draw processed hours later reads higher MPV than the same blood measured promptly — no biology involved. Platelet clumping (an in-tube artefact some people's blood is prone to) distorts MPV and the count both. A surprising MPV on a single draw, with a normal count and no symptoms, is usually just repeated — and often normalises.
When to talk to a doctor
- An abnormal platelet count — that's the number that leads; MPV is its supporting clue.
- Bleeding or bruising symptoms — easy bruising, bleeding gums, nosebleeds, petechiae (tiny red skin dots) — regardless of what MPV says.
- A persistent, unexplained trend across repeat tests.
- An isolated off-range MPV with a normal count and no symptoms — in most cases, a footnote rather than a finding.

The reverse combination, low platelets with small ones, points more toward the marrow struggling to produce. Either way it's the count, symptoms and trend that drive what happens next, not the MPV alone.
What does a low MPV mean?
Smaller-than-average platelets — an older platelet population or a marrow producing less briskly. It can accompany marrow suppression (some medications, chemotherapy, aplastic conditions) and some chronic inflammatory states. Like high MPV, a lone low value with a normal platelet count and no symptoms is usually just noted, not chased.
Does an abnormal MPV mean cancer?
No. MPV is not a cancer test, and an isolated high or low MPV is a poor reason to worry about one. Rare marrow disorders can change platelet size, but they announce themselves through the platelet count and other CBC lines — which is exactly why MPV is read as part of the panel, never alone. If your count is normal and you feel well, an off-range MPV is overwhelmingly likely to be normal variation or a sample artefact.
Can the MPV result be wrong?
Yes, more easily than most values. Platelets swell in the standard EDTA blood tube over time, so a sample that waited hours before analysis reads a falsely high MPV. Platelet clumping (an in-tube artefact some people's blood is prone to) skews both the count and MPV. This fragility is one more reason a surprising MPV on one draw is usually just repeated.
Should I worry about an abnormal MPV?
By itself, rarely. The questions that matter: Is your platelet count normal? Do you have symptoms (unusual bruising, bleeding gums, petechiae, fatigue)? Has the value changed persistently across repeats? Three no's make an off-range MPV a footnote. A clearly abnormal count, symptoms, or a persistent trend is a clinician conversation — with MPV as one supporting clue among several.
